Terms of Service

1 Introduction & Acceptance of Terms

These Terms of Service govern the admission, clinical care services, residential accommodations, and website usage of AgeWell Senior Care & Rehabilitation Centre, located at 14A Southern Avenue, Kolkata. By booking a consultation, submitting an application, or admitting a resident, the resident and their legal guardian agree to abide by these terms.

2 Admission Criteria & Pre-Assessment

• All prospective residents undergo a clinical pre-admission assessment by our geriatric medical team to determine appropriate care level (Assisted Living, Transitional Rehab, Palliative, or Dementia Care).
• Recent hospital discharge summaries, medical prescriptions, and pathological reports must be presented prior to admission confirmation.
• AgeWell Care does not admit individuals with active severe infectious communicable diseases or acute psychiatric violent instability requiring closed asylum care.

3 Medical Authorization & Emergency Protocols

In the event of an acute medical emergency, AgeWell Care is authorized by the resident’s guardian to initiate emergency first-aid, administer on-site life-support protocols, and promptly transfer the resident to a pre-designated tertiary hospital in Kolkata.

All emergency hospital hospitalization expenses, ambulance transit fees, and intensive care tariffs are the direct responsibility of the resident’s primary guardian.

4 Billing, Payments & Security Deposits

• Care package fees are billed in advance on a monthly basis and must be settled within the first 5 days of every billing cycle.
• A refundable security deposit is maintained during the resident’s stay and refunded within 14 working days after final discharge clearance.
• Individual medicines, external doctor consultations, and specialized diagnostics are billed at actual cost.

5 Visitor Code of Conduct

Family members and visitors are welcome during designated visiting hours (Mon-Sat 8:00 AM - 8:00 PM, Sun 9:00 AM - 5:00 PM). Visitors must register at reception, observe hygiene protocols, respect other residents' peace and rest schedules, and follow staff safety instructions.

6 Discharge & Termination of Service

Voluntary discharge requires a 15-day prior written notice by the legal guardian to facilitate medical summary preparation and medication handovers. AgeWell Care reserves the right to terminate residency in circumstances of non-payment or severe breach of code of conduct.

7 Jurisdiction

Any disputes arising out of or in connection with these terms shall be subject to the exclusive jurisdiction of the competent courts in Kolkata, West Bengal.
